A witness testimony is something which a qulified colloeague you work with can write. It's a brief statement which says they have seen you carrying out this particular performance criteria, how you did it & that you followed the settings practices & procedures for this correctly.
A personal statement/reflective account is a statement you write yourself where you say exactly what you did & how you did it. You could also 'reflect' on your practice i.e saying if you think you did it well & what you could do differently next time or if you need further training or guidance from a colleague to do this better next time.
Your assessor or college centre should have told you about these forms of evidence & how you can & shouild use them to provide evidence & support the completion of elements & units.
Witness testiomonies & personal statements/reflective accounts should also be able to be used to provide evidence for some of the knowledge for each unit. You shouldn't need to write 'answers' for every point!
