I'm confused Help Please !!!!!!!!!!!!!!!!!!!

Hello

I am going through slowly my second module and I have been slowly reading this chapter and I am writing up my fourth question and I don't know what they mean

Explain when and why inquiries and serious reviews are required and how the sharing of the findings informs practices

I think this could mean looking up some cases of child abuse and neglect and explain what happened and what the government and social services could have done to stop the death of the child or children involved but I don't know if I have got myself confused.

Could someone tell me if I have got this wrong please ?
