I have a question Evaluate the ways in which concerns about poor practice can be reported.

Hi all,
My assessor has resubmitted my answer for this question.
She would like me to explain the following:
Is it easy to do?
How effective is it to find out about procedures?
As well as to give the positive and negatives of this?

My minds gone blank!
