(
pg 101) serious case reviews are undertaken by a Local Safeguarding Children Boards after a child dies and abuse or neglect is beleived to be a factor. In addition to this, serious case reviews are considered in all cases where
- a child has sustained a potentially life-threatening injury though abuse or neglect, or
- a child has sustained serious and permanent impairement of health or development through abuse or neglect, or
- important lessons, for inter-agency working could be learnt.

page 103 of this EYE handbook that will help - search inside for 'case reviews inform practice'
Can you check with your tutor if the criteria you've posted actually means you need to look for a SCR that's happened within your county (search online using
serious case reviews and your
county name) rather than
within your provision/the setting where you work? As whilst there are sadly a number SCRs each year it is less likely your setting has been involved with one. You can read about
whistleblowing examples in EY settings on page 100/101
Serious Care Reviews inform practice as the key prurpose of a SCR is to enable lessons to be learnt as widely and thoroughly as possible. Professionals need to be able to understand fully what heppened in each case, and most importantly, what needs to change in order to reduce the risk of such tragedies happening in the future.
