SOS Unit 3.14

Hello my lovely Silky's.
So I am coming to the end of my course and am a little stumped. I have been asked the following:

Show how you have worked with colleagues, parents/carers to plan next steps.
Guidance
Using the observations from Q7 show how you have planned the next steps for:

• an individual child
• a group of children

Include details of how you planned this and who was involved e.g. parents/carers, colleagues, or SENCO. Upload the plans using the supporting evidence section.

You should have a minimum of 2 plans each plan should be a minimum of 200 words.

I know what they want to see and I get what I need to write but I am a little baffled as to how to format it as they are stating I need to quote "plan"
any template would be much appreciated :)
