How serious case reviews inform practice?

Explain the impact serious case reviews have on all stakeholders involved in safeguarding children.

I think this is the right one....

This is what i have put...

EYE22-4.1 Explain why serious case reviews are required.


Safeguard children, staff, parents and carers is paramount, within serious case reviews current practice is examined, what's happening and how it happens, within the setting and externally with other organisations.

Practitioners are given the opportunity to pass information via their meeting agendas or promotion of their open door policy for raising concerns (whistleblowing) about practice or other issues, that anyone feels has the potential to contribute or course harm to children. It helps everyone look at information and sharing it, storing it, transporting it, data protection and freedom of information act, the referral, reporting procedures and contact lists that are in place.

Serious case reviews reflect a setting's practice and has recommendations that would improve safeguarding measures. Settings will be able to identify any issues and see ways to implement, discuss and adapt on, ensuring a setting continually improves what it does to safeguard well-being and welfare.

Local safeguarding board considers conducting serious case reviews where;
• A child sustains a potentially life threatening injury through neglect or abuse.
• A child has been subjected to particularly serious sexual abuse
• A case raises concerns about inter-agency working to protect children from harm.

The purpose of a SCR is to
• To clearly identify what these lessons are, how they will be acted upon and what is expected to change as a result.
• to improve inter-agency working to safeguard and promote the welfare of children
• Establish whether there are any lessons to be learnt from the case about inter-agency working.

EYE22-4.2 Analyse how serious case reviews inform practice
The main purpose for a serious case review is to learn lessons from cases where a child has been seriously harmed, died and abuse or neglect is known or suspected to be a factor.
In order for lessons to be learnt as best as possible professionals need to fully understand what happened in each case, and more importantly what needs to be done to ensure they reduce the risk of it happening again .
Serious case reviews help to put policy’s in place to help safeguard children in the future, one of these is a mobile phone policy, settings must have a place where all staff place mobiles in a locked box at the beginning of their shift to ensure it can not be used around children.
