Hi, welcome to the site, as you collect evidence from your working practice you will need to check that all the criteria have been covered.

Work place evidence can be
Direct observation of learner by assessor
Professional discussion
Expert Witness evidence
Learner’s own work products
Learner log or reflective diary
Activity plan or planned activity
Observation of children, young people or adults by the learner
Portfolio of evidence
Recognition of prior learning
Reflection on own practice in real work environment
Written and pictorial information
Scenario or case study
Task
Oral questions and answers
Assessment method developed by your course provider

You don't mention the task details or the evidence that you submitted so maybe read back over what was asked of you and then see how your work product did and did not proove your knowledge & understanding.

Best wishes

ahh right, there's a page that helps explain reflective accounts here and an activity planner here, many of the work products you'll collect will be the formats that your setting or placement already have in place eg. observation sheets, policies & procedures, professional development plans and self appraisals, incident & injury recording sheets.

If you're still unsure please get in touch with your course provider, it's their duty to support you through this and will be able to offer you advice on the type of evidence they're expecting you to produce xx

Hth, best wishes
