Hi, referrals are made between agencies or service providers when one offers a level of support that another is currently unable to.
Do you know if your local authority has a multi-agency referral panel or other systems in place for a
TAC team around the child approach? These are ways for providers to be known in an area and how services can be secured for individuals & their famillies.
Professionals include:
Health Visitor
Social worker
School attendance officer
School nurse
Psychologist
Youth worker
In general referrals are made when there are concerns about a child's development, their welfare, learning and when moving between service providers. Practitioners, parents, carers, the child or young person themself can make a referral, by phone, email or letter. Each referral must be followed up.
How referrals are made can be unique to your county, the setting and who its associated partners or agencies are.
If you're in England the
CAF and pre-caf forms support an assessment of needs and are something that's used directly with the child, young person & family themselves so that contacts with others can be agreed & established with the aim that outcomes will improve once services have been put in place.
If it helps, can you ask in your setting what would happen if a child's speech, language and communication appeared delayed or not developing quite as expected. or if a child's behaviour was giving cause for concern, who would you speak to?
There's good reading in the CYP3.6 chapter if you've access to a handbook?
Best wishes xx
