Default Parental permission form for observing their children

Hi everybody,
Does anybody out there have a link or template for a "parental permission form, for observing their children"?
I am doing the foundation degree and have to observe children, one assignment is a child in play, unstructured, and the other in class setting with teacher. As i work in a Primary school i wouldn't need a form usually for daily observations as i do them all the time, however, for uni i have to have the permission form and reply to show as part of the evidence.

I would be grateful for any help offered.

I think I'd just write a letter to the parents explaining what you are doing and why, with a section for them to sign/agree, keep a signed one for evidence but give the parents a copy to keep too.

Check confidenciality too - no visable name/address etc

I thought that's what i would end up doing, but hoped somebody might have had a template. Maybe i'll do one.

Perhaps something like this....

Setting Name





Child’s Name


I give permission for my child’s key person/worker to carry out observations on my child, these can be written, photographic or video. They can be used by my child’s key person/worker to record my child’s achievements, learning and development and to plan his/hers next steps.

I give permission for these observations to be shown to a College/University Tutor as evidence of my child’s key person/worker’s practice.

I understand that these records will be available for me to look at on request.
