Hi, a warm welcome to silkysteps.
This thread may help with
writing a reflective account. It sounds like your teacher is aware of your role in the assessment process, is it possible you can ask them to help identify the times assessment accurs?
eg. record keping and how you use what you already know about child's learning to support new learning, strategies and how you interact with individual children differently to suppot unique learning needs, deployment - where you go to be with the children during times of learning, how you engage and invite other children to examine learning together, how you share outcomes - achievments and guidance for changes, the time you give to help children reflect on thier learning, how you share observations about the knowledge children had and what they know now. Identifying learning needs will depend on the child, what you're covering and what you're using to record it
This unit from C&G might be helpful reading -

3.2 Listen carefully to learners and positively encourage them to communicate their needs and ideas for future learning
3.3 Support learners in using peer assessment and self-assessment to evaluate their learning achievements
3.4 Support learners to:
a reflect on their learning
b identify the progress they have made
c identify their emerging learning needs
d identify the strengths and weaknesses of their learning strategies and plan how to improve them.
