Default Collate information about learners English and Maths skills?

Does anyone have any idea how i can answer the following question?
Collate information about learners english and maths skills ; Learning targets ; English and Maths support needs
Any ideas would be much appreciated...i am very confused on what i have write.
Is it the way i am assessing their skills during a planned activity?

Hi, welcome to the site. This specific criteria needs you to gather up infomation about the children's current learning targets and any support needs they have for English and Maths.

This links directly to what happens in your setting so you are best placed to check with your class teacher.

If it helps there's good information about the criteria on page 128 of the STL handbook

It begins by saying how you will need to show your assessor evidence about how you support English and maths in a range of contexts across the curriculum, and goes on to say;

LO1 Be able to identify learner needs for English and maths

AC 1.1 Collate information about learners' English and maths skills : learning targets : English and maths support needs


Pupil's learning targets
Most pupils will have specific targets in English and maths that they will need to bear in mind when working on learning activities. Pupil'#s learning targets will be linked closely to assessment for learning so that they know what they are working towards and can assess for themselves against this; older pupils may set their own targets, whish will give them more ownership over their learning. It is important when setting targets that these are clear and achievable, and can develop pupil's confidence to help them to progress. Their targets should be recorded somewhere so that they are to hand when pupil's are working - where are they kept in your class?

Pupil's English and maths support needs
Teachers should be able to talk to you about pupil's support needs in amths and English. For example, they may need specific help in the area of calculations if they are learning a new way of working in maths, or need support when problem solving.
