Default Collate information about learning targets that support children's maths and English skills

Does anyone have any idea how i can answer the following question?

Collate information about learners english and maths skills;

Learning targets
English and Maths support needs

Any ideas would be much appreciated...i am very confused on what i have write.
Is it the way i am assessing their skills during a planned activity?

Hi You'll need to collect information from your setting, your class teacher should be able to provide you with all the details.

The following is a quote from Unit 8, page 128+ of this level 3 STL handbook

AC 1.1 Collate information about learners': English and maths skills; learning targets; English and maths support needs

In order to provide evidence for this unit, you will need to show your assessor how you support English and maths in a range of contexts across the curriculum. However before you can do this you will need to identify their needs for English and maths support. As you speak to teachers and tutors and get to know pupils, you will find out about their needs and abilities in each area. If you are supporting pupils during learning activities, you should also be able to gain additional information nabout learners' English and maths skills, learning targets, and English and maths support needs.

Targets
Most pupils' will have specific targets in Englkish and mathsthat they will need to bear in mind when working on learning activities.

Pupils' learning targets will be linked closely to assessment for learning so that they know what they are workling towards and can assess themselves against this; older pupils may set their own targets, which will give them more ownership over their learning. It is important when setting targets that these are clear and achievable, and can develop pupils' confidence to help them to progress. Their targets should be recorded somewhere so that they are to hand when pupils are working - for example, in the front of an exercise book or folder.

Support
Teachers should be abvle to talk you about pupils' support needs in maths and English. For example, they may need specific help in the area of calculations if they are learnign a new way of working in maths, or need support when problem solving.

Did you guys manage to do this question? Wondering if you would be up for sharing what you wrote so I can see how best to write it, I'm totally thrown by it.
My class teacher gave me a really brief answer but basically told me to speak to a TA and she's always busy so not been able to catch her for a chat so hoping I can answer it with the knowledge I already have.

Hi, the key word in this criteria is collate, you need to bring together the information about how your class teacher uses learning targets so that you're able to support your children's math and English skills.

Dfferent teachers may do this differently so you will need to ask what happens in your class or use the knowledge you already have about how targets are used.

The next criteria asks you to explain them, so for this one you're just collecting information.

How are targets used in your setting?
Are they written on planning sheets?
Are you expected to fill out any information after an activity?

These are quotes from p.128:

Research it:
Find out about the kinds of targerts pupils might have for maths and English at each key stage. You can include targets for the age range you support.

In Practice:
Make sure you are fully aware of the maths and English skills, support needs and learning targets of the pupils you support so that you have these to hand when you are working with them. You can choose a group you support and record these yourself, or use working documents from your everyday practice to show your assessor.

I'm confused Help!!!

Can anybody please share with me how they actually wrote their answer for this...I just don't know where do begin! Oh...and I'm new on here so I have no idea whether this post will be in the correct place but I'm looking for help for: Collate information about learners English and Math skills; learning targets; English and Maths support needs.
