Hi, the names of individuals and organisations will very depending on your local authority and setting but the refferal flow chart at the bottom of London's Child Protection website is most useful - http://www.londoncp.co.uk/chapters/referral_assess.html

If possible ask your setting how their multi-agency process works xx
