Unit 17 Need help!!!

I have 3 units to do then I have finished. But i am so stuck....

[17.1] Understand how to place children and young people at the centre of assessment and planning

2.3 explain how the goals and targets identified will support the achievement of positive outcomes.

2.5 explain how to support a child or young person and others to understand and agree the goals, targets and outcomes of the plan

Please help me, I am so stuck on all the unit, but kinda hoping if i can get ideas for this one then maybe the rest will come flowing.
