Help SOS Parent/ carers feedback form ?

Hi

I was just wondering if anyone can tell me where I find a parent/ carers feedback form to save me from starting from scratch.

I have found the questions for cildren so I assume that there is a standard form in existence for parents?

There isn't a standard form that I am aware of in existence anywhere.

I use one I have designed myself that follows the Every Child Matters agenda.

I ask the question and give parents some ideas of the things they should be thinking about when they answer.

I sent them to 7 parents (I am a childminder) at Christmas with children's home made cards, little presents made and wrapped by the children, bills and newsletters.

Despite gentle prodding, I have only received one back so far. Anyhow she's happy so I should be grateful. She wrote 'ok' in each of my boxes!

I am thinking about following a new career as a forger

I have made one but it isn't in word so im not sure how to share it. Be careful how you give them the chance to score, on a 1-10 people are less likely to put 10 and so we did poor fair average good and very good, luckily we got lots of very goods !

We tend to change ours depending on what we want feedback on. For instance the last one we sent out was about the parents helpers role. We have also sent one out about enrolement. We do send general ones out but find parents only really bother to fill them in when they think they have an issue. So we listen to the playground gossip and send them out as required

We sent one out recently asking for parents views on the preschool in general, snacks etc etc, and we sent a blank sheet for their comments. We have had quite a few back - all positive!

Maybe use that for a base then anything that is coming back as a potential problem, follow that up?
