Default Unit 2.5 help needed please

Hi all,
I have hit a brick wall I think! Was doing so well with this assignment!

Question 3.4
Analyse the potential tension between maintaining confidentiality and the need to disclose information:
• When poor practice is identified
• Where a child is at risk
• Where it is suspected that a crime has been/may be committed

The book has very limited explanations of this and I have no idea if I need to write more than the book has, or just reword it?

Also ...
Question 4.1
Demonstrate and record in a chart how your setting encourages parents/carers to take an active role in their child’s play, learning and development
I dont understand how to demonstrate this, its not a practical question, and what sort of chart.
