Think about when a child first starts at the school - what information is needed eg name, address, contact telephone numbers, emergency contact numbers, date of birth, ethnicity, religion, language spoken at home, any allergies, special diets, special educational needs, medical problems, behavioural problems, communication problems eg deafness, muteness, delayed speach? etc, etc......
My setting also likes to see reports from previous setting if any eg nursery or previous school which give useful information about a child's development and capabilities.
When a child first starts at my setting, we do baseline assessments to find out their abilities eg do they know their colours, shapes, numbers, can they write their name, can they spell basic CVC words, what level reading are they at, what is their writing like eg joined up, single spaced, back to front, what is their understanding of the English language if English is not spoken at home? How much English do they speak?
Also teachers do SATS tests at the end of each key stage so they can assess children's level in reading, writing and numeracy.
