im so close to finishing but struggling with last few questions....

Hi please I need help:( I'm stuck on unit 17
2.3 explain how to support a child or young person and others to understand and agree the goals, targets and outcome of the plan
3.2 agree with a child, young person and others how goals and targets will be measured

hi i just gave an example of what i do in my worksetting.

When a young person is newly referred i would read their core assessment , a taking good care would have or will be completed on admission (parent/carer would be there). All relevant information would also have been discussed in staff meetings or handovers eg likes, dislikes, behaviour, medication or any allegies. Each child has their own link worker who would put together a healthcare/placement plan and also a careplan - to suit the childs individual capabilities and needs. This would be then placed in the child's individual file for other staff and professionals to read when coming into contact with the child.

I then gave an example of one of our young persons file and explained what their goals, target and outcome are.

Hi sorry I didn't reply sooner stopped doing my course work for awhile had builders in and also had a family bereavement so I couldn't get back in to my course but I'm back on track now:jump::jump:
