Default CYP Core 3.1 Supporting Transitions 16 - 19 years

Hi, I have completed all of the chart apart from 16 - 19 years old transitons, i am really struggling with this one. Pleaseee helpp!

Thanks i just need the support given now i have only really done about the transitions from school because thats all i need to do.
