These are the records that patents complete for you and you complete for them. Do at their basic, they could be the initial information forms from parents, telling you about any allergies or specific needs. They may tell you about the child's routine through the day and week, what specific words they may use to alert you to things like the toilet etc. they may talk about ways to pacify their child if upset. They give you details of emergency contacts and permission to administer medicine. Things like that. Then there's the paperwork that you provide for the parents, the baseline/two year assessments, the ongoing assessments and next steps sharing. These are discussed with parents regularly, discussions recorded and parent and practitioner signs. That's the sort of thing I'd be looking for in answer to this question
