not sure if this will help, but this is what we have at work and Ofsted read them a few weeks ago

Notifiable diseases
Under the Reporting of injuries, Diseases and Dangerous Occurrences Regulations 1995 (RIDDOR’95) which came into force on 1st April 1996 Pre-schools will also have to report any outbreaks of notifiable diseases to RIDDOR. Details on reporting to RIDDOR are outlined below.

Internet by completing the relevant form on www.riddor.gov.uk
Form by completing the relevant hard copy form on www.riddor.gov.uk
By facsimile 0845 3009924
By post to:
Incident Contact Centre,
Caerphilly Business Park,
Caerphilly,

Acute encephalitis
Acute poliomyelitis
Anthrax
Cholera
Diphtheria
Dysentery
Food poisoning
Leptospirosis
Malaria
Measles
Meningitis
Meningococcal
Pneumococca
Haemophilus influenzae
Viral
Other specified
Unspecified
Meningococcal septicaemia (without meningitis)
Mumps
Ophthalmic neonatorum
Paratyphoid fever
Plague
Rabies
Relapsing fever
Rubella
Scarlet fever
Smallpox
Tetanus
Tuberculosis
Typhoid fever
Typhus fever
Viral haemorrhagic fever
Viral hepatitis

Leprosy is also notifiable, but to the director, CDSC.

If you have two or more cases of gastro-enteritis you need to inform Ofsted.
