Communication Activity sheet!

Does anyone no how/have an example of an communication activity sheet? I need to see what one looks like so i can desgin my own. My tutur couldn't find me on to show me.

she has drawn me on my sheet that they write on with the work that they want you to do.

So I am taking it that you just need to think of an activity that promotes the use of communication and literally put the info in in under the headings. Place each heading into a box and write the relevant info in each one. You could use hand drawn sketches to demonstrate/simplify the instructions of, your method and maybe even photographs of the activity taking place in the evaluation section.
Recommendations can be drawn from the do's and don'ts that you discovered worked/didn't work when carrying out the task.
I doubt very much that there is just a 'standard' sheet for this.
