Default behaviour/intervention policy

would anyone have a policy they could share which addresses what happens when behaviour cant be remedied in the setting? for example if a child needs withdrawing for the safety of others or if beh mod isnt working or being supported by parents?
i was thinking it could be included in the behaviour policy?
not something we like to think about but we need to have one.
