Hi, it is confusing, having existing knowledge and experiences of assessing needs helps enormously, the evidence you have within your setting will vary depending on how they've accessed services in the past, are in contact with now, their approach and feelings about multi-agency, integrated working.

The majority of the work we do with children, young people and their families is done with their agreement. At the start of our involvement, we seek the "consent" of the parent or young person to share information with colleagues in other agencies. Whether we seek the consent of the parent, or the young person or, in certain circumstances, act without their consent are all issues covered by information available from:
How would consent be taken? is there a permission slip, signature area on a form, how did you arrange for child, parent, young person to actually make an informed mark agreeing to build the content of an action plan?
page 6 states:
- We need to ensure that we seek the views of the child or young person and their parents and that their views inform any formal Action Plan.
How would that be recorded? are notes taken, is information clarified, through a meeting maybe where child/ parent/ young person are all present. Is there a letter that is sent to confirm dates, times, venues.

CAF Action Plan:
The purpose of assessment process is to agree an "Action Plan" involving the Team Around the Child and the child / young person and their family.
The Action plan records: "What needs to change", "who will do it", "by when" and "how will you know when things have improved?" The Action Plan should also indicate when a review meeting will take place.
Are child/parent/young person supported to be the most active element of identifying what needs to change, do they know 'who is, is everyone comfortable with the 'by when and how - the method of reporting back so everyone is kept upto date.
Do you have any hard copies of the setting's policies and procedures that are given to child, parent, young person that explains the process involved with organising help and support.
Does child / parent / young person have preffered service providers or knowledge that guides preferences, are they able to supply you with any literature/pamphlets?
