Outcome 1
expects you to .. Understand how to place children and young people at the centre of assessment and planning

-- This can happen by being able to assess situations and prioritise children's needs over other commitments or
    expectations.
-- by knowing what your current paperwork & information systems are - how do you assess and how do you plan?
-- recognise and value children's interests - how do you use observations - types and development plans in the setting?
-- by being able to organise meetings, catch-ups, time to speak with children in places they can suggest, access at
   suitable and appropriate times
-- by asking children for their views, opinions and inviting feedback
-- by listening to their ideas and suggestions
-- by understanding their perspective and wanting to provide a service that meets their need and  improves their life
   chances and positive outcomes

1.1 Explain the value of a child centred model of assessment and planning
 yellow star There is value in knowing how a child centered model can focus in on an individual person, their unique set of situational circumstances and how it enables tailored support and specific sourcing of external service providers to happen. A child centered model promotes a child's right to choice, communication and to express themselves. Where children are actively encouraged to take the lead this model would hope to see them flourish creatively as they explore possibilities, make connections they feel are relevant to them and formalise decisions that affect their lives in a positive way. As with A Gibbs reflective cycle assessment is part of a planning cycle

1.2 Explain how to identify the needs of children and young people

Assessing needs depends on the professional's reason for undertaking an assessment, how to assess depends on methods already in place, formats or layout of paperwork.

1.3 Explain the importance of working with others to assess the needs of children and young people
       to inform planning

      of children and young people of all ages and levels of understanding in assessment and planning

       expressing their needs and aspirations to inform planning
2.2 Demonstrate how to encourage a child or young person to plan realistic, achievable but
       challenging goals
2.3 Explain how the goals and targets identified will support the achievement of positive outcomes
2.4 Develop a plan with a child or young person and others to meet their needs with a focus on the  
       achievement of positive outcomes
2.5 Explain how to support a child or young person and others to understand and agree the goals,
       targets and outcomes of the plan



Outcome 3 expects you to ..  Be able to work with children and young people to implement the plan for the achievement of positive outcomes

3.1 Demonstrate how a child, young person and others are provided with clear information about the
       roles and responsibilities of all those involved in the plan
3.2 Agree with a child, young person and others how goals and targets will be met
3.3 Demonstrate how a positive approach is used to encourage children or young people work
       towards the achievement of goals and targets
3.4 Demonstrate how progress towards goals and targets is agreed and recorded with the child or
       young person

 Outcome 4 expects you to ..  Be able to work with children and young people to review and update plans

4.1 Demonstrate how to record relevant information to prepare for a review
4.2 Demonstrate how to support children and young people to contribute to reviewing achievement of
       goals and targets
4.3 Demonstrate how to contribute to reviews based on measurement of progress of the child or
       young person
4.4 Present information to reviews about aspects of the plan that are working well and those that need        to be changed
