The best way I have found to do it is to follow the Every Child Matters agenda and give parents examples of what they might say.

So for healthy child I ask them to comment on our menus, healthy eating ethos, the themed activities their child has taken part in, cooking activities, dental hygiene etc.

Hth

dont make it too long or complicated.. that way you get more replies.. if it is too much they just will not bother. we kept ours to 2 pages and felt that was too long for some.
