I need to get together some sort of application form, to find out childrens details, g.p, allergies etc, please help
how can i do this?

Yes NCMA have an 'all about me' type form or you can make one yourself, there are lots of ideas if you search through Google.

make sure it reads well and is easy for parents to complete - you must have it finished before you start working with the children.
