look into the children's act 2004 every child matters this might give you some ideas or the Death of Victoria Columbie. These would be good examples of why there are serious case reviews and will explain what a serious case review is.
Lynn.

info for task 2, hope this can ge you started x

. The importance of partnership working runs through every aspect of safeguarding from government legislation to local working. It is important we communicate and co-operate with agencies or groups involved in the welfare of a child/ young person to promote the well being. Partnership working in every context means different organisations working together such as state funded bodies and businesses. In the context of safeguarding it means establishing and abiding by guidelines and working within the law, especially with the regard to the protection of children. The most new development to help agencies and professionals to share information is the (CAF) common assessment framework system this enables multi agencies to access and add information about a child’s needs

A serious case review will always be undertaken when a child dies, and when abuse or neglect is the suspected cause of a child’s death. The local safeguarding children board (LSCB) will also consider whether a serious case review should be conducted where a child has been seriously injured through abuse or neglect or been subjected to a serious sexual assault. and they are done to see what areas can be improved, look at baby p case for example and talk about the outcome of that case.

This explanation on education.gov.uk also outlines what serious case reviews are:
http://www.education.gov.uk/a0068869/scrs

Quote:
Serious case reviews are undertaken by Local Safeguarding Children Boards (LSCBs) for every case where abuse or neglect is known or suspected and either:

• a child dies; or
• a child is seriously harmed and there are concerns about how organisations or professionals worked together to safeguard the child.

Additionally, LSCBs may decide to conduct a SCR whenever a child has been seriously harmed in any of the following situations and the case gives rise to concerns about the way in which local professionals and services worked together to safeguard and promote the welfare of children (including inter-agency and inter-disciplinary working):

• A child sustains a potentially life-threatening injury or serious and permanent impairment of physical and mental health and development through abuse or neglect.
• A child has been seriously harmed as a result of being subjected to sexual abuse.
• A parent has been murdered and a domestic homicide review is being initiated under the Domestic Violence Act 2004.
• A child has been seriously harmed following a violent assault perpetrated by another child or an adult.
Why case reviews are needed is to learn the lessons that a case presents - failings, negligence, gaps and how better to protect children.
