Referral to a nursery from a social worker

hi all

I'm in the middle of a partnership assignment at the moment. For task 2 it has some cases studies and I'm a little stuck on case study 2 basically the question is

Lucy is a one year old, she has started to attend your nursery 3 days a week following a referral from a family social worker.
What information/documentation may Lucy have in place?

I'm really stuck the only thing I can think of is medical files but there isn't a reason as to why she's being referred thanks for your help in advance xx
