Default CYP3.2 Assessing children

I have the following assessment task to do and I am having to do this through a theoretical approach as I'm a nanny and currently work with a 9 month old, my assessor has asked me to do it as a child going to nursery.

If anyone has any helpful comments i would really appreciate it.

"Choose a child who soon will be experiencing a transition in their lives. Maybe going to school or going to a new nursery.
You need to prepare the following prior to your assessor’s visit. The information can be collated in any format.
Assess her/his development in the following five areas: physical, communication, intellectual/cognitive, social/emotional/behaviour and moral
Explain what factors you took into account when assessing development
Explain the selection of the assessment methods used
Develop a plan to meet the child’s development needs, showing how the plan is child centred and inclusive and why this is important. Explain the features of the environment that promote the development of the child and how your setting is organised to promote children’s development
Implement the plan, taking into account that development is holistic, ensure you listen to the child in a way that encourages them to feel valued and encourage them to actively participate in decisions affecting their lives and the services they receive according to their age and abilities.
Please make sure you have covered all learning outcomes 1.1, 1.2, 1.3, 1.4 2.1, 2.2, 2.3, 2.4, 2.5, 3.1 3.2, 5.1, 5.2, 6.1, 6.2."
