Default short and medium planning

I have been asked by my assessor to think of a theme and to do a medium plan and then a short plan and after that I have to evaluate it too and I am going stir crazy as I just cant get my head round it if someone could help I would really appreciate it and even if anyone has a blank copy of a medium or short plan that i could complete it with a few hints along with it and yes I know this is alot to ask and very cheeky but i really need some help as my assessor is coming wednesday afternoon PLEASE HELP

Plan one actitivity based on your theme..... first decide which of the EYFS areas you want to cover...... link it to a specific development matter..... e,g under Physical Development " using one handed tools".... then for a medium term plan the rest of the theme and how you will develop it over a longer period of time. We are doing Spring so I planned for one weeks activities (short term) which was about flowers and blossom limked them to the EYFS and then for our medium term developed other areas baby animals, life cycles e.g caterpillars and butterflies etc. Hope i explained it ok????
