Help SOS *witness testimony and professionally discussion*

Hi everyone ive just got 5 weeks left to finish off my TA level 3. i wanted to find out if any one has done a professional discussion with their assessor through oral and written questions and if you anyone has an example of this so i can make up my own one to complete my questions i have left.

also if anyone has use evidence from a witness at the setting and what format this needs to be in as my class teacher has agreed to do it but we are unsure of the format for a witness testimony and what should be included any help will be appreciated.

Hi usually they give you the paperwork for you witness information to go onto. Mine is an evidence form that has to have the name and signature of the witness etc on it also. Ask your tutor for this information or google it. I have also just got a plain piece of paper that has been written on and dated and signed etc.
