Default assessment for learning

Hi everyone i am hoping someone may be able to give me examples of what they are looking for to help get me started. I really feel that my head is going to explode i have all of outcome 3 and 4 to do before wed and am feeling really stressed. I know i need to do this myself but even just a push in the right direction will help i know this is how i talk to children, listen to them, get ideas that would benefit them, give feedback etc i just can't seem to write it down on paper sorry to ask but im reallt struggling xx

3.1 Use information gained from monitoring learner participation and progress to help learners to review their learning strategies, achievements and future learning needs
3.2 Listen carefully to learners and positively encourage them to communicate their needs and ideas for future learning

Hi, would it help to tackle this by asking

How you do monitor learner participation?
How do you monitor progress?
How do you review learning strategies?
Review achievements?
Review future learning needs?

3.2 links to the above as listening to learners informs the review process..
How do you encourage learners to communicate their needs and their ideas for the future?
