Default Ensure. Effective practice in the use of records or reports for communication

I am struggling with this part of the unit .pc 28 -41
I am having to write about when I did an incident report and using legal , work setting and inter agency policies and procedures for accessing and completing records and reports . Clarify for others the legal and work setting and inter agency policies and procedures for accessing and completing records and reports. Provide evidence for your judgment and decisions within records and reports including where this is based on informed opinion. Record evidence which clarifies and support your judgment and decisions. Record evidence which conflicts with your judgment and decisions . Produce records and reports that encompasses best practices, positive achievement and outcomes for children .Produce records and reports that are accurate concise , objective, understandable and legible. Ensure that information in records and reports is accessible to children and in a form appropriate to their communication needs and preferences. Where records and reports are to be used for decision making, confirm their accuracy and the accuracy of accompanying evidence with all those they concern. Secure any signatures that are required. Document any conflicts,disagreement, unmet needs of risks with recording and reporting. Encourage those who use your records and reports to discuss feedback with you . Take action in response to feedback from those who use your records and reports. Ensure records and reports are stored and shared within confidentiality agreement and according to legal and work setting and inter agency agreements and requirements.

I know there is lot I just need a help on how to do as its about an incident report and contacting the social services and telling them what's on my incident report the thing I am lucky enough not yet have to have dealt with it and maybe something on the children's care plans and safeguarding issues or a child with additional support needs and evidence that affect decisions making on the child's life or children's panel so don't know the policy and procedures what to write and record who what to write, record who what and why information can be shared. If anyone has dealt with this in their work setting any help would be greatly appreciated.
