I'd try to get to get a good image in mind of the playroom / whole establishment - where tables, tops, doors, carpets, sinks etc are, or a floor plan and then very slowly work through each point ...
For number 2 I'd use deployment, responsibilities and staff preferences as a guide .. 3 would be provision of resources that support and promote development & risk taking .. climbing, jumping, balance, role play, games with & without rules .. thinking continually about what was done in an own placement and how it happens ..
