Assessing and recording outcomes

Can anyone help, I need to give descriptions of Development matters, Planning and resourcing, Effective practice, Look, listen and note. Is there a definion I should be using or referring to or do I just write what I think it means? :dizzy: this is driving me mad :bawling:
