progress checklist

hi can anyone tell me where i can get a 2 year progress check sheet from that ofsted will be happy with, or are we expected to do our own,if so can anyone tell me what is expected to be on the sheet.
want to get it right as got my ofsted inspection very soon and don't want to mess up.
