Hi, sorry for the late reply. If it still helps page 144 of the EYE handbook states

Reporting lines

For partnership reporting to be effective, there have to be clear lines of reporting. This ensures that information is given to those who need it and that information does not go astray. It is usual, for example, for children's key persons in early years settings to share and exchange information on a day-to-day basis with parents. On the other hand, where there are concerns about a child's welfare, information will be exchanged between the early years setting's designated person and other professionals.

Then page 121 offers a table where you might see similarities in who you are expected to report to in your workplace/setting.

Staff member: Student on placement/ volunteer
Responsibility: to follow policies or procedures and to summon further assistance in the event of an accident, incident or emergency.
May need to report to: whoever is acting as placement supervisor or, in the event of an emergency, any mem,ber of staff.

Staff member: member of staff with a current paediatric first aid qualification.
Responsibility: to provide immediate assistance in the event of a medical emergency. They may provide direction to others - for example, ask them to call for an ambulance.
May need to report to: medical teams in the event of a child needing medical attention.

Staff member: health and safety/ fire officer
Responsibility: to ensure that the health and safety/ fire policy is being followed and to advise the manager as to how to make improvements. Should act on reports by members of staff about faulty equipment and potential hazards.
May need to report to: depends on setting but usual;ly deputy, manager or head teacher

Staff member: deputy
Responsibility: to take on the responsibilities oin the absence of the manager. To take specific responsiibilities for health and safety in some roles.
May need to report to: mananger

Staff member:
Manager/head teacher
Responsibility: to ensure the smooth running of the setting. To ensure that all policies relating to health and safety are being followed.
May need to report to: Parents in the event of a serious injury, incident or emergency

Staff member: director/owner/governors
Responsibility: to take on the legal responsibility in the event that an accident, incident or emergency is the fault of the setting
May need to report to: Heathy and Safety Executive , Ofsted


Check with your setting to make sure you know what your reponsibilities are when it comes to reporting concerns.
